The Donna's Kids Financial Assistance Application for Team Fees Financial Assistance

This application is designed to help parent/guardian who,financially with the player's team approved budget fees. This application does require the team budget to be attached for review by the Director of Donna's Kids.

NOTE: Applicants who received assistance in the 2024/2025 season and received a team year end refund due to a team account surplus, must have submitted the refund back to the Donna's Kids program. Any player refund from the 24/25 season not returned, may be deducted from the requested team funds for the 2025/2026.

2025/2026 Team Fees - Should there be any refund at the end of the 2025/2026 season received by the parent/guardian of the player in this applicant, the funds will be submitted back to the Donna's Kids Program by way of payment to the Kitchener Minor Hockey office.

ALL Applications will be monitored and reviewed with a response with 3-5 business days.
