The Donna's Kids Financial Assistance Application for Hockey Registration has TWO parts for parents to complete.
1. DK Application Below
Upon completing this application, a confirmation email will be sent. The Director of Donna's Kids will contact you to arrange a meeting time via phone or in person. Please be aware that supporting documents to verify the application information will be required with the application prior approval.
Director of Donna's Kids will be contacting regarding your registration for financial assistance between 3 to 5 business days.
2. Hockey Registration through KMHA Online Registration
Once the application has been reviewed an email will be sent to the parent/guardian with the status of the application (approved, pending or denied). If approved, the email will provide further details regarding how your child will be registration for the season and any balance owing for the registration. THIS IS A NEW PROCESS for previous years due to the KMHA online registration processes changing for the 24/25 season
Please Note: Should your child require Team fees assistance or Equipment, there will be separate online forms to complete and the applications will be available under Financial Assistance Application tab on the home page. Thank you
